Insert human judgment at explicit risk and uncertainty boundaries.
"A human is involved" is not enough. The system must define when the human is asked, what evidence is shown, what decision is available, and what happens after approval, rejection, correction, timeout, or escalation.

Show the proposed action, supporting evidence, expected impact, alternatives, and a clear diff. Avoid approval fatigue: if reviewers approve hundreds of meaningless prompts, the control becomes ceremonial.
A human click is not proof of safety. Reviewers need enough time, context, authority, and an easy way to reject or edit the proposal.
